PDA-Informed Autism Assessments for Ages 8+
Exploring the drive for autonomy and demand-avoidant patterns within a comprehensive autism evaluation. Also known as Pathological Demand Avoidance (PDA).
"I Want to Do It, but My Brain Refuses"
You know the email will take five minutes. You may even want to send it. But the moment it becomes an expectation, an invisible wall goes up. Bills, phone calls, plans with friends, even things you love can start to feel impossible once they turn into demands.
This isn't laziness or stubbornness. For people with a PDA profile, everyday demands can register as a threat, and avoidance is the nervous system's way of staying safe. Many have spent years being called defiant, difficult, or unmotivated, while privately feeling panicked and ashamed.
Haven Health offers neuro-affirming, PDA-informed autism assessments in Vancouver, WA, for children, teens, and adults ages 8 and up across the Portland metro, in person or by telehealth. Dr. Lana Ferris and Dr. Martha Keith are AuDHD clinicians with experience identifying PDA profiles, and our process is built to respect your need for autonomy rather than fight it.
What "PDA-Informed" Means
PDA is not a standalone diagnosis in the DSM-5-TR or ICD-11, and research on it is still developing. Clinicians and the autistic community describe it as a profile within autism, originally named pathological demand avoidance and now often called a pervasive (or persistent) drive for autonomy.
So we don't offer a separate "PDA diagnosis." Instead, your comprehensive autism evaluation looks closely at demand avoidance, anxiety, and your need for control. If you are autistic and a PDA profile is present, your report describes it and tailors its recommendations to it, so you, your family, your employer, or your school understand which kinds of support help and which backfire.
Signs of a PDA Profile
A PDA profile can look very different from the autism stereotype, especially in adults who mask. People often recognize several of these:
Avoiding Everyday Demands
Ordinary tasks, and sometimes things you want to do, become impossible once they feel like an expectation.
Anxiety, Not Defiance
Demands trigger panic or a sense of threat. The avoidance protects you; it isn't a choice to be difficult.
Social Strategies to Deflect
Charm, humor, excuses, distraction, or negotiation used to sidestep requests, often so smoothly that no one notices.
A Strong Need for Control
Hierarchy, micromanagement, and rigid schedules feel intolerable, while autonomy lets you thrive.
Burnout When Autonomy Shrinks
High-demand periods lead to shutdowns, meltdowns, or burnout where even hobbies feel like chores.
A Rich Inner World
Fantasy, roleplay, or imaginative worlds offer a space where you have complete control.
Why PDA Profiles Are Often Missed or Misread
Many children, teens, and adults with a PDA profile are first labeled with oppositional defiant disorder, a mood disorder, anxiety, or a personality disorder. Others mask so effectively that their struggles stay invisible: they excel at work or school, then crash at home.
Standard autism advice can also backfire. Visual schedules, firm routines, and reward systems add demands, which can increase anxiety for someone with a PDA profile. An assessment that recognizes the profile helps point you and your supporters toward low-demand, autonomy-respecting approaches instead.
