Superbills Provided

We provide superbills for you to submit to your insurance for potential out-of-network reimbursement.

Self-pay means your diagnosis stays private - no insurance records or prior authorizations needed.

We Are an Out-of-Network Provider

Haven Health Autism Assessments does not contract with any insurance companies. This means that all patients are self-pay patients, regardless of your insurance status. We provide a superbill upon request that you may submit to your insurance for potential out-of-network reimbursement, but payment is due in full at the time of service.

Important Notice for Medicaid/OHP Members

Please be aware that Oregon Health Plan (OHP) does not cover autism assessments for adults. Additionally, regarding Washington State Medicaid (Apple Health/Molina), we have been unable to locate any providers in the state who accept Medicaid for adult autism assessments. Because there are no insurance-based options available for these specific services, paying out-of-pocket is often the only route for adult diagnosis.

Good Faith Estimate

Under the No Surprises Act, you have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost.

If you are uninsured or choose not to use your insurance, you can request a Good Faith Estimate before your appointment. This estimate will include the expected charges for your visit and any follow-up care.

For more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
